STAR Awards Nomination Deadline and Instructions
Applications are due by 4:30 p.m., Monday, March 17, 2008. There are two ways to receive a STAR award! Nominations are accepted for each of the categories listed below. An individual may also self-submit an application for the following categories: Promotion of Diversity, Innovation by a Team, and Outstanding New Program. An individual, team, or program may be considered for one award category only. Additionally, you must submit one application form per category of nomination or self-submission (for example, if you are nominating two different employees for two different awards, you must use one application form per award nomination). Two letters of support, in addition to a letter of nomination from the person making a nomination or self-submitting, must be included with the application packet. Supporting materials (program brochures, pictures, etc.), if desired and appropriate, should be forwarded with the application packet and fit into an 8 ½” X 11” envelope.
